    All-on-4® – 4-Implant Prosthesis

    All-on-4® is a full-arch restoration protocol designed for patients who are completely edentulous or whose teeth require extraction. The entire prosthetic structure, which can restore up to 12 to 14 teeth in a single arch, is supported by only four implants thanks to the strategic placement of these implants in the jawbone.

    The key to this method is biomechanics. Two anterior implants are placed vertically in the anterior region, where bone preservation is typically best. Two posterior implants are placed at an angle of approximately 30 to 45 degrees, allowing for the utilization of available bone, avoiding the maxillary sinuses and alveolar nerve, and evenly distributing chewing forces along the entire arch. This arrangement ensures structural stability and, in most cases, eliminates the need for bone augmentation, which not only shortens treatment time but also significantly reduces its overall cost.

    At Focus Clinic, All-on-4® is conducted within the proprietary Focus System protocol by a team of implant surgeons with hundreds of successful full-arch procedures under their belt. All stages, from Focus 360 diagnostics with 3D tomography, through implant placement, to the final prosthetic restoration in the Perfect Lab, are handled within a single organization. This means the surgeon, prosthetist, and prosthetic technician work in direct collaboration, ensuring the restoration is precisely tailored to your face and bite. A dedicated Patient Advisor oversees the logistics of the entire process, appointment scheduling, and treatment financing options (MediRaty, Confinio, PayDent).

    All-on-4 Treatment

    A unique feature of All-on-4® is the immediate loading protocol. In a single procedure, the surgeon places four implants, and the prosthetic team installs a temporary full-arch restoration on them the same day. This means that the patient does not leave the clinic without teeth, even if they arrive completely edentulous or after extractions requiring extraction. For many patients, this single day is a turning point, when they regain chewing function, natural speech, and a smile they have lacked for years.

    The temporary restoration serves for the period of osseointegration, which lasts 4 to 6 months. Healing time depends on the implant location; the mandibular bone is denser and heals faster than the maxillary bone. During this period, the patient functions normally, eating, speaking, and working. Dietary modifications are required in the first 2 weeks and regular follow-up visits are scheduled 7, 14, and 30 days after the procedure.

    After confirming full osseointegration, we take digital impressions and design the final full-arch restoration in our Perfect Lab laboratory. The final design differs significantly from a traditional acrylic removable denture, resting on a metal core (usually titanium alloy) covered with a highly aesthetic veneering material. This design is lightweight, mechanically durable, and resistant to cracks and wear.

    At Focus Clinic, we offer three material variants for the final full-arch restoration. Acrylic restorations are the basic solution, offering excellent aesthetics and functionality. Ceramic-metal restorations offer higher durability and color stability over time. Highly aesthetic porcelain restorations provide the most natural appearance of teeth and the highest level of aesthetics, comparable to premium restorations.

    The functional effect of All-on-4® is completely different from traditional dentures. The patient regains approximately 80 percent of the chewing force of natural teeth, while a classic removable full denture provides only about 30 percent of this force. The lack of an artificial palate allows for the full experience of food flavors, and the permanent attachment of the structure eliminates the discomfort of a removable denture, the need for glue, and the risk of the work falling out while eating or speaking.

    Who is All-on-4® for?

    This method is designed for patients who are completely edentulous in the upper or lower jaw, those whose teeth require extraction due to advanced periodontal disease or extensive caries, and those who are tired of full removable dentures and wish to transition to a permanent restoration. All-on-4® also works well for patients with moderate bone loss, for whom traditional implant placement would require bone expansion and extend treatment by many months.

    Why are the posterior implants in All-on-4® placed at an angle?

    The angle of placement of the posterior implants serves two purposes. First, it avoids important anatomical structures, such as the maxillary sinuses in the upper jaw and the alveolar nerve in the mandible, making the procedure safer and avoiding the need for sinus lift. Second, the angled placement utilizes bone where it is better preserved and extends the implant-to-bone contact area. This means greater stability and the possibility of same-day placement of a temporary prosthetic.

    How long does the full All-on-4® treatment process take?

    The time from the initial consultation to the placement of the final restoration typically ranges from 5 to 8 months. The first 2 to 3 weeks involve the Focus 360 diagnosis and treatment planning. On the day of surgery, we place the implants and secure the temporary restoration. Osseointegration takes 4 to 6 months, with healing typically faster in the lower jaw than in the upper jaw. The final 4 to 8 weeks are devoted to the design, fabrication, and placement of the final full-arch restoration.

    Does All-on-4® eliminate the need for bone grafting?

    In most cases, yes. Thanks to the angled placement of posterior implants, the All-on-4® method allows for the utilization of bone where it is preserved and avoids atrophic areas and anatomical risk zones. This allows the patient to avoid months of bone augmentation procedures and sinus lifts. In extreme cases of extreme jaw bone loss, we consider treatment with zygomatic implants, which bypasses atrophic areas to an even greater extent.

    What is the difference between All-on-4® and All-on-6?

    The main difference is the number of implants supporting the full-arch restoration: four in the All-on-4® and six in the All-on-6. All-on-4® is a sufficient solution for most patients, even those with moderate bone loss, and allows for reduced surgical intervention and treatment costs. We recommend All-on-6 for patients with particularly high occlusal loads (e.g., bruxism), with very good bone quality allowing for additional implants, and for patients prioritizing maximum long-term stability.

    Does All-on-4® have an artificial palate?

    No. The stability of the structure is provided by implants, not by support from the mucosa, so the artificial palate typical of removable dentures is not necessary. The absence of a palate means full perception of food taste and temperature, free articulation, no gag reflex, and significantly greater comfort compared to a traditional full upper denture.

    What are the recommendations after the All-on-4® treatment?

    After the procedure, you will receive a package of recommendations, a prescription for painkillers and an antibiotic, and follow-up appointments at 3, 7, 14, and 30-day intervals. For the first two weeks, you must follow a soft diet (soups, purees, eggs, fish, cooked vegetables), avoid chewing hard foods, avoid intense physical activity, and refrain from saunas, swimming pools, and sunbathing. You will maintain oral hygiene as instructed by your doctor. Your patient care provider will remain in contact with you throughout your recovery.

    How much does All-on-4® treatment cost at Focus Clinic?

    The cost of All-on-4® treatment consists of two stages. The surgical stage, including temporary restoration, ranges from PLN 25,000 to PLN 30,000. The prosthetic stage, including the final restoration, depends on the selected material and ranges from PLN 25,000 to PLN 27,000 (acrylic restoration), PLN 34,000 to PLN 36,000 (ceramic-metal restoration), and PLN 38,000 to PLN 40,000 (highly aesthetic porcelain restoration). An implant consultation costs from PLN 150 to PLN 200, and you receive a detailed treatment plan free of charge after the Focus 360 diagnosis.

    Can I pay for All-on-4® in installments?

    Yes, at Focus Clinic, we offer three forms of treatment financing: MediRaty, Confinio, and PayDent (deferred payment). All-on-4® treatment is a long-term investment, so paying in installments is often crucial when deciding to begin treatment. Your Patient Advisor will help you choose the optimal financing option and plan a payment schedule according to the stages of treatment (separately for the surgical phase and the prosthetic phase).
