An overdenture is a modern removable prosthesis that, unlike a traditional fixed denture, is attached to implants or the patient’s own teeth using special attachments. This provides stability similar to permanent solutions while remaining a structure that the patient can remove independently for daily hygiene.
This is a compromise between two worlds. On the one hand, it eliminates all the major problems of traditional removable dentures, such as movement while eating and speaking, the need for adhesives, sore mucous membranes, and the gag reflex associated with an artificial palate. On the other hand, it is significantly less expensive than a fixed full-arch All-on-4 or All-on-6 reconstruction and requires less surgical treatment, as stabilization is usually achieved with two implants (an economical solution) or four implants (a more comfortable solution).
Overdentures are designed for patients who have been wearing full or partial removable dentures for years and are not comfortable with them, for patients who wish to replace their traditional dentures with a solution that provides reliable stability, and for those for whom All-on-4 or All-on-6 treatment is currently beyond their financial or medical reach. Overdenture – Treatment Process
A key element of an overdenture is the attachment system, which determines its stability, comfort of use, and daily care. At Focus Clinic, we use three basic systems, individually selected during the treatment planning phase after the Focus 360 diagnosis.
The first system is ball-type attachments (locators), the most popular and economical option. A small, spherical head is attached to the implant, and a nylon or silicone matrix is placed in the corresponding position inside the denture. Ball-type attachments are a simple, intuitive solution, easy for the patient to use daily. They require periodic replacement of the attachment inserts (usually every 1 to 2 years), which wear out mechanically due to repeated removal of the denture.
The second system is a retention bar (bar). A metal bar is placed between the implants, onto which special brackets engage in the denture. This solution provides the best stability of all overdenture attachment systems, as the work is supported not only by the attachment points but also by a bar that distributes chewing forces. A bar requires a larger number of implants (usually four) and more complex laboratory work.
The third system is telescopic, which consists of primary crowns placed on implants or the patient’s retained natural teeth, over which a prosthesis with secondary crowns is attached. The telescopic system provides superior chewing comfort, excellent force transfer, and the ability to use the patient’s retained natural teeth as additional supports, which is particularly important in cases of partial tooth loss.
The entire treatment process, from Focus 360 diagnostics (3D tomography, digital intraoral scan) through implant placement, to the completion of the prosthetic work in Perfect Lab, is conducted within a single organization. This means direct collaboration between the surgeon, prosthetist, and dental technician, faster turnaround, and precise fit of the prosthesis to your mouth and facial features. How does an overdenture differ from a classic removable denture?
A traditional removable denture rests solely on the gums and palate, which can make it unstable, requires adhesives, and limits taste and chewing efficiency. An overdenture is stabilized on implants or natural teeth, providing a secure fit, requiring no adhesives, and preventing it from falling out during eating or speaking. Full-arch versions can have a reduced artificial palate, allowing the patient to regain full taste and temperature perception.
How is an overdenture different from an All-on-4 and an All-on-6?
An overdenture is a removable structure that the patient removes for hygiene purposes, typically mounted on two or four implants. All-on-4 and All-on-6 implants are fixed structures, screwed onto four or six implants, which the patient does not remove. Overdentures are less expensive, faster to complete, and less invasive, but require daily removal for cleaning. All-on-4 and All-on-6 are more advanced, fixed, and provide a feel similar to natural teeth, but require a greater surgical and financial investment. The choice of method is discussed individually.
How many implants are needed for an overdenture?
The number of implants depends on the chosen fixation system and the patient’s bone conditions. The most economical and quick solution is to mount the prosthesis on two implants with ball-and-socket attachments. The four-implant option provides better stability and is recommended for patients with higher occlusal loads or those seeking maximum comfort. With a retention bar, we most often use four implants. The decision is made after analyzing a 3D tomography scan.
Does an overdenture have an artificial palate?
In the variant supported by four implants or a retention bar, an upper jaw overdenture can be constructed without an artificial palate, as the implants themselves provide stability. This means full taste sensation, freedom of articulation, and no gag reflex. In the variant with two implants, the artificial palate can be partially preserved for increased stability, which is discussed individually during the treatment planning phase.
How often do I need to replace the mounting tabs?
Snap-in inserts (usually made of nylon or silicone) wear mechanically from repeated removal and insertion of the denture, so they typically require replacement every 12 to 24 months. The replacement itself is performed during a short follow-up visit and requires no intervention in the implants or the denture structure. The retention bar and telescopic elements are much more durable and do not require regular replacement, other than periodic adjustments.
Will I receive my teeth on the day of the procedure?
For overdentures, we typically use a staged treatment protocol rather than immediate loading. This means that the implants first require 3 to 6 months for full osseointegration, and only then is the final prosthesis with a fixation system fabricated. During the healing period, the patient can use their existing removable prosthesis (after proper adjustment) or a prepared temporary fixed prosthesis. Immediate loading for overdentures is applied individually, after analyzing the bone conditions.
How long does the entire overdenture treatment process take?
The time from the initial consultation to the placement of the final overdenture typically ranges from 4 to 7 months. The first two weeks involve a Focus 360 diagnosis and the development of a treatment plan. Next, we place 2 or 4 implants under local anesthesia, after which the implants require 3 to 6 months for osseointegration. The final 3 to 5 weeks are devoted to fabrication of the prosthesis at the Perfect Lab, installation of the fixation components, and precise adjustment of the structure.
How to care for an overdenture?
Care includes removing the denture daily and thoroughly cleaning it with a special denture brush and dedicated products (not regular toothpaste). Additionally, we recommend soaking the denture in a cleaning solution once a week. Implants and the attachments in the mouth should be cleaned with a soft brush and dental floss. Regular checkups every six months allow for the assessment of the implants’ condition, replacement of the retaining inserts, and professional cleaning.
Does an overdenture prevent bone loss?
Yes, this is one of the key advantages of this method. Implants stabilizing overdentures transfer chewing forces to the jawbone in a manner similar to natural tooth roots, thereby stimulating bone tissue and preventing further atrophy. In comparison, a classic removable denture only loads the mucosa and does not transfer forces to the bone, which accelerates the process of alveolar ridge resorption.
What are the contraindications to overdentures?
The main contraindications include uncontrolled diabetes, advanced osteoporosis (which hinders implant integration), untreated periodontal disease, active oncological treatment, and blood clotting disorders. Severe bone loss that prevents the placement of even two implants also precludes the traditional method, but in such situations, we consider alternative solutions. Smoking significantly reduces the chances of treatment success. Full qualification is performed after 3D tomography analysis.
How long does an overdenture last?
With proper hygiene and regular checkups, implants that stabilize dentures last 20 to 30 years or longer, and the implants are covered by a lifetime manufacturer’s warranty. The denture itself (an acrylic plate with teeth and attachment sockets) has a lifespan of approximately 7 to 10 years, after which it is recommended to replace it due to natural wear and tear of the material, changes in the contour of the alveolar process, and the natural wear and tear of acrylic teeth. Snap-in inserts should be replaced every 1 to 2 years.
How much does an overdenture cost at Focus Clinic?
An overdenture costs from PLN 20,000, and the final price depends on the number of implants placed (2 or 4), the chosen fixing system (ball clasps, retention bar or telescopes) and the material of the final prosthesis.
