Teeth in a Day on Zygomatic Implants
Zygomatic implant placement is the most advanced procedure in implantology and requires both an experienced surgical team and precise preoperative diagnostics. Therefore, at Focus Clinic, every treatment begins with a comprehensive Focus 360 protocol, a key element of which is a 3D computed tomography (CT). This allows for a precise assessment of the anatomy of the zygomatic bone, the position of the maxillary sinuses, and the course of the nerves, as well as planning the precise path for each implant.
Depending on the patient’s situation, we use two basic treatment options. In cases with bone loss limited to the posterior region, we use two zygomatic implants in the posterior region of the maxilla, supplemented with two standard implants in the anterior region, where bone loss is usually preserved. In extreme cases, where bone loss encompasses the entire jaw, we place four zygomatic implants and base a full prosthetic restoration on them.
The procedure is performed under local anesthesia with sedation or general anesthesia with an anesthesiologist, depending on the scope of the procedure and the patient’s preferences. The entire procedure typically takes 2 to 3 hours. In most cases, we use immediate loading, meaning that the temporary implant is attached to the implants on the day of the procedure. The patient leaves the clinic with teeth that restore chewing function, clear speech, and a beautiful smile. The entire treatment is sometimes referred to as “teeth in a day.”
The temporary implant serves for the period of osseointegration, which lasts 4 to 6 months. During this time, the zygomatic implants fuse with the zygomatic bone, creating a durable, biomechanically stable support. After this stage, we take digital impressions and design the final prosthetic restoration in our Perfect Lab laboratory. CAD/CAM technology, our own laser milling machine, and direct collaboration between the surgeon, prosthetist, and technician within a single organization allow us to precisely control every step of the process.

How do zygomatic implants differ from traditional ones?
Three key differences are length, anchorage, and application. Traditional implants are 8 to 15 millimeters long and are implanted into the alveolar bone of the maxilla or mandible. Zygomatic implants measure 30 to 55 millimeters long and anchor into the zygomatic bone, bypassing areas of bone loss. They are used only in cases of extreme bone loss in the upper jaw and always as part of a full-arch restoration, not for individual gaps.
Why are zygomatic implants only used in the upper jaw?
Zygomatic implants are anchored in the zygomatic bone, which is located on the sides of the face, at the level of the upper jaw. The mandible lacks a similar bone structure that could serve as an anchor point for implants of such great length. Furthermore, the mandibular bone is naturally denser than the maxilla and is much less susceptible to atrophy that would preclude traditional implant placement. Therefore, standard implants or the All-on-4 technique are usually sufficient in the mandible.
Do zygomatic implants allow you to avoid bone grafting?
Yes, this is one of the key advantages of this method. Traditional treatment protocols require augmentation of native or xenogenic bone and sinus lift for significant bone loss, which extends treatment by up to 9 to 12 months. Zygomatic implants bypass areas of atrophied jawbone and are anchored directly to the zygomatic bone, eliminating the need for bone grafting.
How many zygomatic implants are needed to rebuild a full arch?
The number of zygomatic implants depends on the extent of bone loss. In the Zygoma Hybrid variant, we place two zygomatic implants in the posterior region of the maxilla and supplement them with two standard implants in the anterior region, where bone is usually preserved. In the Quad Zygoma variant, used for extreme atrophy of the entire maxilla, we place four zygomatic implants, which themselves support a full-arch restoration. The choice of variant is determined after 3D tomography analysis.
Is zygomatic surgery safe?
Zygomatic implants are a surgical procedure that requires a highly skilled team, but with proper planning and execution, they are very safe and predictable. Safety is ensured by 3D computed tomography (CT) for precise implant planning, the use of surgical guides, the operator’s experience, and strict sterility protocols. Potential risks, such as maxillary sinusitis, are minimized through thorough preoperative diagnostics and appropriate antibiotic therapy.
How long do zygomatic implants last?
With proper oral hygiene and regular checkups every six months, zygomatic implants last 20 to 30 years or longer. The implants themselves are covered by a lifetime manufacturer’s warranty. Clinical studies published in the implantology literature indicate that the survival rate of zygomatic implants is comparable to that of standard implants. The final prosthetic restoration requires periodic servicing and can be replaced without damaging the implants themselves.
What are the contraindications to the procedure?
The main contraindications include uncontrolled diabetes, active oncological treatment of the head and neck, severe blood clotting disorders, active maxillary sinusitis, and certain autoimmune diseases. Smoking significantly reduces the chances of treatment success. Pregnancy and breastfeeding mean the procedure should be postponed. Full qualification is performed during a consultation at Focus Box, following a 3D tomography scan and an assessment of the patient’s general condition.
What are the recommendations after zygomatic surgery?
After the procedure, you will receive a post-procedure package, a prescription for painkillers and antibiotics, and a schedule of follow-up appointments at 3, 7, 14, and 30-day intervals. For the first two weeks, you should follow a soft diet and avoid strenuous exercise, nose blowing, saunas, and swimming pools. During the healing period, we recommend not wearing removable dentures that will rest on the treated area. Your patient care provider will remain in constant contact with you throughout your recovery.
Is zygomatic treatment painful?
The patient feels no pain during the procedure, as the procedure is performed under local anesthesia with sedation or general anesthesia. In the postoperative period, for the first 3 to 7 days, facial swelling, bruising, and pain discomfort may occur, which are effectively controlled with prescription painkillers. Most patients return to normal activities within 7 to 10 days after the procedure.
How much do zygomatic implants cost at Focus Clinic?
Zygomatic implant treatment (surgical phase with temporary work) costs from PLN 45,000. The prosthetic phase, i.e., the creation of the final full-arch work, costs from PLN 30,000. An implant consultation costs from PLN 150 to 200, and you receive a detailed treatment plan free of charge after the Focus 360 diagnostics. The final quote is provided after 3D tomography analysis and a decision on the treatment option (Zygoma Hybrid or Quad Zygoma).
