Custom implants (also called subperiosteal implants) are precisely designed titanium structures that are not screwed into the bone like a traditional endosteal implant, but are attached to the cortical bone and placed between the bone and the gum. Each implant is created exclusively for a single patient, based on a custom 3D model of their jaw, using CAD/CAM technology and 3D printers, using medical-grade titanium.
This solution is dedicated to patients with extreme bone loss who cannot benefit from either traditional endosteal implants or even zygomatic implants. The most common indications include complete atrophy of the alveolar process after years of edentulism, post-craniofacial trauma, advanced periodontitis, osteoporosis, and chronic sinus problems (polyposis, recurrent sinusitis), which prevent a sinus lift. In such situations, traditional bone augmentation procedures are either impossible or carry too high a risk.
The main advantage of custom implants is that they are designed precisely for the individual patient’s anatomy – bone geometry, nerve path, sinus position, and oral structures. This allows them to bypass anatomical limitations and allow for the reconstruction of a full dental arch, even where other methods have failed. How a Custom Implant is Made – From 3D Design to Implantation
Custom implants are distinguished by a fully digital production process that begins with a precise Focus 360 diagnostic. A key element is 3D CBCT, which allows us to obtain an accurate three-dimensional model of the patient’s bone. Based on this, we use specialized CAD/CAM software to design the implant geometry precisely tailored to the patient’s anatomy, taking into account the thickness and quality of the cortical bone, the position of nerves, the structure of the maxillary sinuses, and the planned prosthetic restoration.
The completed design is then sent to production on a 3D printer, where the implant is manufactured layer by layer from medical-grade grade 5 titanium. This process ensures precision unattainable with mass-produced implants while also achieving a surface with properties that promote bone integration. Before the implant is delivered to the patient, it is sterilized and undergoes quality control.
The custom implant procedure is a single-phase procedure. This means that during a single procedure, the surgeon inserts the implant and secures it to the cortical bone, most often using miniature stabilizing screws, without disturbing areas of atrophied alveolar bone. The procedure is performed under local anesthesia with sedation or under general anesthesia with an anesthesiologist’s assistance, depending on the scope of treatment and the patient’s preferences.
One of the key advantages of this method is the very quick path to a permanent prosthetic restoration. Unlike traditional endosseous implants, which require 3 to 6 months of osseointegration, custom implants are manufactured with primary stability allowing for the placement of the final prosthetic structure within a few days of the procedure. For patients with long-term edentulism, this means a return to full chewing function and a natural smile in days, not months.
The final prosthetic restoration (a full-arch crown or bridge) is designed in our Perfect Lab using CAD/CAM technology, using premium materials such as solid zirconia or porcelain on a zirconia base. Direct collaboration between the surgeon, prosthetist, and technician within a single organization allows for control over every stage of the work. Custom implants are designed as a plate or frame tailored to the patient’s anatomy, attached to the cortical bone, and can be used in both the maxilla and mandible. In clinical practice, the method is selected based on 3D tomography analysis. In some cases, custom implants are the only option when zygomatic implants are insufficient or contraindicated.
Do custom implants eliminate the need for bone grafting?
Yes, this is one of the key advantages of this method. The implant is designed to rest on the existing cortical bone, without the need for reconstruction. How long does the entire treatment process with an individual implant take?
The time from the initial consultation to the placement of the permanent prosthetic restoration typically ranges from 6 to 10 weeks. The first 2 to 3 weeks include Focus 360 diagnostics, digital implant design, and 3D printing production. After implant placement, the permanent prosthetic restoration can be placed within a few days. The final 4 to 6 weeks are devoted to the design and fabrication of the final prosthetic restoration in the Perfect Lab laboratory.
What are the contraindications to treatment with an individual implant?
The main contraindications include uncontrolled diabetes, active cancer treatment, advanced autoimmune diseases, severe blood clotting disorders, and uncontrolled maxillary sinusitis. Smoking significantly reduces the chances of treatment success. Pregnancy and breastfeeding mean the procedure should be postponed. Full qualification is performed during a consultation at Focus Box, following a 3D tomography scan and an assessment of the patient’s general health.
How long do custom implants last?
With proper oral hygiene and regular six-monthly checkups, custom implants can last for many years. The medical-grade titanium from which they are made has a proven long-term durability in implantology. The final prosthetic implant requires periodic servicing and can be replaced if necessary without damaging the implant itself. Care and adherence to the scheduled checkups are crucial to longevity.
What are the recommendations after the procedure?
After the procedure, you will receive a package of recommendations, a prescription for painkillers and an antibiotic, and follow-up appointments at 3, 7, 14, and 30-day intervals. For the first two weeks, you must follow a soft diet, avoid intense physical activity, saunas, swimming pools, and nose-blowing. You will maintain oral hygiene as instructed by your doctor, and your Patient Care Assistant will remain in constant contact with you throughout your recovery.
How much does treatment with an individual implant cost?
Due to the fully individualized design and manufacturing process, the price of treatment with subperiosteal and custom implants is determined individually for each patient. The final cost depends on the scope of reconstruction, bone anatomy, the type of final prosthetic work, and the complexity of the procedure. An implant consultation costs between 150 and 200 PLN. You will receive a final written treatment plan after the Focus 360 diagnosis.
Can I split the payment for treatment?
